Surgical fees are available at the time of your consultation. We do accept most major credit cards. Patients paying for their surgery with a credit card will be required to show a US government-issued ID with the credit card before your surgery can proceed.

Scheduling and cancellation policy

We require a $125 (surgical) or $75 (non-surgical) non-refundable consultation fee due when scheduling an appointment (must be paid prior to appointment date). The fee will be applied towards your surgical or cosmetic procedures or can be used as credit towards our retail products.

Our goal is to provide quality, individualized, medical and cosmetic care in a timely manner. Please remember that we have reserved appointment times especially for you. In an effort to provide prompt service, we request at least a 48-hour advance notice in order to reschedule your appointment. This gives us the opportunity to open up the time for other patients.

For cancellation or rescheduling please call to cancel via phone at (408) 728-8887 – If we are unavailable feel free to leave a detailed voicemail. Alternately, cancellations or reschedules may be texted to the same number. Messages received will be responded to during normal business hours. If the appointment must be canceled, the non-refundable consult fee will be credited to your account with us at Chase Lay MD & Associates, Inc. if canceled before the 48 hours. However, if you do not cancel prior to the 48 hours of the cancellation period, the payment will be forfeited.

For existing patients that do not cancel or reschedule prior to the 48 hours, we will require that you repay the $125 consultation fee to schedule your next appointment.

For surgery appointments:

Cancellations made 2 weeks before the procedure date will receive a refund of any payment made toward surgical fees, minus the deposit of $750 and the cost of any implants or materials purchased for your surgery. Rescheduling is permitted with at least 2 weeks advance notice at no charge for the first occurrence. Any additional rescheduling will be charged a fee of $250 per occurrence.

Any cancellations or reschedules made less than 2 weeks in advance will be charged a $250 cancellation/rescheduling fee.

Any cancellations or reschedules made within less than 48 hours notice of your surgery appointment will result in a forfeiture of the $750 deposit. We will need to recollect the $750 deposit to schedule a new date.
